Reach out to TBF to request assistance for the required treatment. Be sure to provide all necessary details regarding your treatment needs by completing this form
4. Await Confirmation
After submitting your request, wait for confirmation from TBF regarding the status of your assistance.
5. Send Receipts and Proof of Payment
Once you have received confirmation and completed your treatment, ensure your receipts meet the requirements sent out below, then email your receipts and proof of payment to [email protected] or click here.
